Rewards Selection Form Terms

Important information – please read this carefully because we need your agreement

In using this Reward Selection Form, to select new options and/or continue with your current benefits and pension contributions, you are agreeing to be bound by the rules of any insurer and/or Envigo terms that apply to each benefit and the pension.  We have produced a benefits brochure “Rewards at Envigo” (available on ukenroll.envigo.com) which should be read together with the details you see on Rewards Selection Form.

One of the most important principles of selecting benefits with Envigo is that on completing this application you are agreeing to exchange pay to cover the cost of some benefits. Some benefits are deducted via salary sacrifice and others are deducted from net pay.  When selecting holiday purchase or pension these will be processed as salary sacrifice which means you are making a temporary contractual change to how you wish to receive pay and benefits (ie until you change benefits selections again or if Envigo changes its offering).

Please note the following

+  Holiday purchase.  This needs to be approved by your Manager.  The Reward Team will seek approval and then confirm this to you by email.

+  Pension.  The choice you have made is a percentage of your Base Salary.

+  Salary Finance, Technology and Cycle to Work benefits are managed direct with the respective provider.

NOTE: If you are on, or are planning to go onto, Maternity leave, Shared parental leave, Adoption leave, Paternity leave or are on Long term sick leave – please refer to Envigo’s HR policies which are held on the Intranet, Quality, Policies, United Kingdom or request a copy from emeabenefits@inotivco.com for confirmation on how benefits and pensions are treated during various types and stages of each type of leave listed.
